I love how this flows. It feels jumbled but not in a bad way, like the way people would think when they're pondering a question.
I'm not entirely certain I'm interpreting it right, but are you trying to say that isolation is the simplest of pronouns - you, I... which divide/isolate us? I wonder if you could make it a bit clearer (although maybe my brain is just slow right now), but I think it's a really clever idea - a nice twist on how we regard the world.
Minor point:
In which case; - I don't think you need a semi-colon here. It's connected to the next line.
